Weather you or your loved ones is living with intellectual and developmental disabilities, in comfort of their own home, transiting from a foster care or unstable housing, in need of supportive housing and daily living assistance, or seeking compassionate and professionally managed group home setting, we welcome your application. Who We Services:

  • Adults 18+

  • Diagnosed with intellectual and developmental disabilities, autism, or dementia

  • Referred by a guardian, provider, case manager, or support agency

  • Require residential support and supervision, or community integration/in-home support

  • Have a funding source or coverage (e.g., Medicaid Waiver, private pay, etc.)

Admission Process:

  1. We’ll conduct a brief screening and assessment

  2. Schedule a tour or intake meeting (if applicable)

  3. Once approved, a personalized transition and support plan is created for a smooth move-in experience.

We make every step clear, supportive, and centered on your loved one’s needs.

    At our center, we are dedicated to providing personalized, evidence-based therapies in a caring and supportive environment. Our goal is to empower individuals and families to thrive in every aspect of life—at home, in the community, and beyond.

    To begin the application process, please gather the following essential documents. These can be uploaded during the application or submitted later if needed:

    • Psychological Evaluation: A formal assessment confirming a diagnosis on the autism spectrum, conducted by a licensed psychologist or developmental pediatrician.
    • Interdisciplinary Team Annual Review: The most recent review document, such as an IEP (Individualized Education Plan), ISP (Individualized Service Plan), or IPP (Individual Program Plan).
    • SIS (Supports Intensity Score) Evaluation: Required for adult applicants only.
    • Medicaid and/or Insurance Information: Documentation to facilitate billing and coverage verification
